2025 FinalForms 

*All parents must create an account and register their students through FinalForms at http://jacksonschools-mo.finalforms.com.  Once an account is made you will complete and sign the checklist forms for your student and upload a valid medical eligibility physical form completed by a healthcare provider.   This must be done before a student can practice and/or try out.

Please make sure the date of examination is completed on the medical eligibility physical form before leaving the physician’s office otherwise the physical will not be accepted. 

You may find a copy of the most up to date medical eligibility physical forms on FinalForms, on the MSHSAA website at https://www.mshsaa.org/SportsMedicine/ under the MSHSAA Resources tab or printed copies can be found in front of the main office.
